Demographic Information needed:
- Name and DOB
- MHA enrolled member
- Verify Insurance
- Phone number
- Drug of Choice (DOC) and last use
Treatment placement priority’s:
- Pregnant women
- IV and fentanyl users
- Opioid and alcohol users
- Incarcerated clients
Step 3
MHA support team will obtain an ROI and contact the treatment facilities for availability and placement.

- Obtain COVID-19 test, evaluation and physical.
